From Hospital to Home: Closing the Gap in Surveillance

Session Description: Home care presents a unique challenge for infection surveillance, particularly for central line-associated bloodstream infections (CLABSIs), where no nationally standardized definitions have been updated for decades. In response to this gap, our infection prevention team developed and implemented a custom surveillance platform and framework to support the consistent identification, review, and prevention of CLABSIs in the home setting.
Now in its third year, this internally developed system enables structured data collection, case tracking, and multidisciplinary event review -establishing a repeatable process in an environment often excluded from traditional surveillance efforts. This presentation will share how the tool was built, what has been learned about infections risks in home care, and how we’ve used the data to engage care teams and elevate line maintenance practices across the continuum of care.
Attendees will leave with practical strategies for building surveillance where none exists, along with insights into balancing clinical judgement, documentation variability, and prevention priorities in complex, non-acute environments.


Laken Chaney

Laken Chaney, MHA, BS, RRT-ACCS, CIC is an Infection Prevention Program Manager at BJC HealthCare, leading initiatives across hospital, ambulatory, and home care programs to improve patient safety and quality outcomes. She holds a Bachelor of Science in Cardio-Respiratory Care from the University of Arkansas for Medical Sciences and a Master’s in Healthcare Administration from Webster University. In 2024, she expanded her expertise with a Certificate in Public Health from the University of Missouri. Laken began her career as a Respiratory Therapist, specializing in ICU care, before transitioning into infection prevention in 2017. She has since developed a robust leadership portfolio focused on CAUTI prevention, high-level disinfection, and environmental services. A Board-Certified Infection Preventionist (CIC), Laken champions advancing evidence-based practices and fostering collaboration among multidisciplinary teams to safeguard patients and healthcare workers. Her passion for innovation and continuous improvement drives her contributions to healthcare-associated infection prevention efforts across her organization.

Kristina Felix

Infection Prevention Manager, Madonna Rehabilitation Hospitals

Kristi Felix, MSN, RN, CRRN, CIC, LTC-CIP, FAPIC. I am the Infection Prevention Manager for Madonna Rehabilitation Hospitals. I have worked in infection prevention in the post-acute setting for 36 years, including LTACH, rehabilitation hospital settings and LTC. I am a Fellow of APIC and earned CIC and LTC-CIP through CBIC. I am a Certified Rehabilitation Registered Nurse through RNCB. I am a Director on the Board for National APIC. I am a Board Member for the Nebraska Infection Control Network and a member of the APIC Nebraska Chapter where I have served on the Board as President, Treasurer, Recording Secretary, and on the education committee. Over the past 15 years I have had the opportunity to serve as National APIC SME and Faculty for various projects with CDC and AHRQ, and was an instructor for the LTC Infection Prevention Certification Course.

Mohamed Elhadidi

Executive Director of infection control , Dallah healthcare

Dr. Elhadidi has worked for more than 15 years in infection prevention and control in the Middle East. (Egypt, Saudi Arabia, Qatar, Oman, Bahrain), earned his MBBCH from Ain Shams School of Medicine and Surgery in 2006 and completed his postgraduate studies in infection control in 2011, is CIC certified since 2015, earned his MBA in risk management in India in 2017, and Master of Science in Health Care Management in Switzerland in 2022 and Board certified in long-term care infection prevention in 2022. Today, he manages the Infection Control Departments at Dallah Healthcare KSA (5 hospitals) and has been an Assessor of Infection Control at the Saudi Central Board for Accreditation of Healthcare Institutions for more than 8 years. He is a tutor for the Saudi MOH IC Diploma and a member of the APIC 2022 ,2023,2024 Annual Conference Committee and chairman of abstract subcommittee for APIC 2025 .
